Passengers leaving Georgia must submit (except for documentation required by visa-free travel) a Passenger Locator Form (PLF) https://app.euplf.eu/#/, Covid-19 green pass, Certificate of COVID Recovery or Certificate of Molecular or Negative Antigen Test result, when entering the territory of Italy.
